Fill out this form to request a room reservation. Your request will be reviewed and a confirmation will be sent to you within 5 days. *PLEASE NOTE: Rooms can only be reserved by St Michael ministries and organizations. *All requests for use of parish facilities to host an event, meeting, etc. included on the Parish calendar, must be accompanied by this form. Upon approval, it will be signed by the appropriate staff member who will notify you of approval/denial based on availability. Any questions please call the parish office at (614) 885-7814.
